Use this form when a player residing in
To obtain Guest Player Passcard, send:
1. Your MSYSA approved Guest Player Loan Form (see above)
2. 1x1" recent color headshot photo in jpeg format
3. Player proof of birth

READ INSTRUCTIONS HERE FIRST. Form required when traveling out of Michigan for a tournament. Those who don't have an assigned "Team ID" simply enter "MSYSA" or "N/A" where applicable.
Tournament Only Teams: Includes an Official MSYSA Approved Roster, Team Official and Player Passcards. Tournament Only Team valid for seasonal year, expiring on August 31st.

Entry Prior to 10 Form: Players who have entered the U.S. prior to age 10. Must supply official documentation. Click here for more information.
International Clearance for Minors: Players who are not eligible for one of the proceeding methods must prove that he/she meets one of these two exceptions: (1) The player has moved with his/her parents to the US for reasons other than playing soccer, OR (2) The player and prospective club are both located within 30 miles of an international border.
