E-Travel Forms
Beginning January 13, MSYSA will be using a new system and new forms for guest players and notification to travel requests. Please read the instructions below on how to obtain each document.1. Guest Player Roster Form (NEW): Use this form when a player residing in Michigan will be playing in a tournament for a team they are not normally registered to. Click Here for Instructions. **If you need a temporary guest player passcard, send your Guest Player Roster form, a 1x1" color headshot photo, a check for $20 (made payable to MSYSA) and a copy of the player's birth certificate to MSYSA at 9401 General Dr. Suite 120, Plymouth, MI 48170. Or, you may stop by during normal business hours with documents**
