Guest & Travel Forms

As of Jan 2012, MSYSA utilizes a new system and new forms for guest players / notification to travel requests. 

Please READ THE INSTRUCTIONS PAGE FIRST before attempting to complete these forms.

Out-of-State players should not use the MSYSA guest player form.  Please visit whatever US Youth Soccer State Association you reside within.

1. Guest Player Roster Form aka Guest Loan Form (NEW): READ INSTRUCTIONS HERE FIRST Use this form when a player residing in Michigan will be playing in a tournament for a team they are not normally registered to. TIP: If you are taking a player not already registered anywhere, simply enter "N/A" or "None" where applicable - this also applies for those who don't have an assigned "Team ID". Non-Members will need to pay a $20 fee to be covered by MSYSA Insurance.

For Temporary Guest Player Passcard(s), send:

    - your completed Guest Player Roster (printed from thetournamentcenter.com website),

    - a 1x1" color headshot photo,

    - $20 Check or MoneyOrder for each card requested (made payable to MSYSA)

    - and a copy of the player's birth certificate or passport

To: MSYSA, Attention Guest Card, 9401 General Dr. Suite 120, Plymouth, MI 48170

or scan all forms and email.  The Credit Card form is required for electronic processing.

You may also bring the forms and payment by the State Office during normal business hours to have a card issued. Cards will NOT be procured if any of the above criteria are not present**

 

2. Permission to Travel (NEW): READ INSTRUCTIONS HERE FIRST. Form required when traveling out of Michigan for a tournament. TIP: For those who don't have an assigned "Team ID" simply enter "MSYSA" or "N/A" where applicable.
