Player/Team Forms
Individual Out of State League Play: For players living inside the State of Michigan and wanting to play in a different US Youth Soccer state association.
People Born Outside of the United States or Non-Residents:First Registration Form: US citizens born outside of the US who have never been registered to play soccer in another country.
Entry Prior to 12 Form: Players who entered the US prior to age 12. Must supply a copy of an official document to prove that fact.
International Clearance for Minors: Any player currently between the ages of 12 to 17 who is NOT eligible for one of the proceeding methods must prove to US Soccer that he/she meets one of these two exceptions: 1) The player has moved with his/her parents to the US for reasons other than playing soccer, OR 2) The player and prospective club are both located within 30 miles of an international border.
