Simply download the form(s) you need, fillout the forms online, then print and mail the forms to MSYSA/ODP 9401 General Dr Suite 120, Plymouth, Mi 48170, unless instructed otherwise. Boys need 4 copies of each form. Make sure each Medical Release Form is notarized. Fill in forms completely.
