Guest Player with Passcard Needed :
If you are going to a tournament and taking a guest player that does not currently have a US Youth Soccer/ MSYSA player pass card, you may request a guest player pass card from the MSYSA State Office. To request a guest player pass card, please read the instructions below.

Request a MSYSA Guest Player Passcard
- Submit the MSYSA Guest Player Roster Form and payment information online for approval
- Print off two copies of the approved MSYSA Guest Player Roster Form
- E-Mail or Mail to the MSYSA State Office:
- One copy of the approved MSYSA Guest Player Roster Form
- Copy of the player’s birth certificate
- 1” x 1” color headshot of the player
Your guest player passcard will be processed within 2-3 days upon receipt of request. The card will be sent back to the address listed for the coach on the approved MSYSA Guest Player Roster form. If you would like it sent to a different address, please indicate where you would like it to be sent.
