For the remainder of the 25/26 seasonal year (until July 31, 2026), please complete the printable form below and email to stateoffice@michiganyouthsoccer.org
Starting with the 26/27 seasonal year (beginning August 1, 2026), please complete the GotSport form linked below.
To download/print a copy of your approved form, go to your personal GotSport account page > Family > choose player’s name in dropdown >Forms > Past Forms and click on the three dots on the far right to ‘View PDF’ of the completed/approved form. There you can Download or Print.
