NEW FOR THE 2026/2027 SEASONAL YEAR:
If your team is traveling on or after August 1, 2026, us the GotSport form below. You will ‘register’ as yourself with your GotSport account and then add team information in the form questions.
To download/print a copy of your approved form, go to your personal GotSport account page > Forms > Past Forms and click on the three dots on the far right to ‘View PDF’ of the completed/approved form. There you can Download or Print.
